Compounded GLP-1: where to get it, plainly compared

Compounded semaglutide and tirzepatide are a lower-cost route to the same active ingredients, made to order by a compounding pharmacy rather than mass-manufactured, and not FDA-approved. It's a real tradeoff people weigh differently. What to know about compounded. Compounded GLP-1 is not reviewed by the FDA for safety, potency, or purity, and since the 2024–25 shortages ended it can generally only be compounded for specific patient-specific reasons. That's the core tradeoff: a lower price for a product the FDA hasn't reviewed. Prefer the FDA-approved brand? See the providers we recommend.

This is our editorial assessment as of June 2026, opinions for your consideration, not statements of fact. We score each provider on a compounded-specific rubric: pricing transparency & value, pharmacy standards, provider oversight, patient support, and shipping. It's a separate rubric from the FDA-approved programs (different products, different criteria), so a compounded /10 isn't comparable to a brand /10. How we score compounded providers

These programs sell a non-FDA-approved drug, so we don't score them against the brand-name programs. They're judged on a separate compounded rubric built for this route. Each earns a /10 on that rubric (its own scale, not comparable to a brand score), and we group them higher- and lower-rated:

  • Pricing transparency & value (30%): is the real ongoing all-in price posted up front? A commitment/volume discount, dose-based tiering, or a clearly-flagged intro promo doesn't count against a provider; pricing hidden until after an intake quiz, undisclosed fees, and cancellation/refund traps do.
  • Medication quality & pharmacy standards (25%): FDA-regulated 503A/503B pharmacy, third-party testing, PCAB accreditation, named pharmacy
  • Provider credentials & oversight (20%): board certification, evaluation quality, monitoring, contraindication screening
  • Patient support & communication (15%): response times, care-team access, side-effect & dose support
  • Shipping speed & reliability (10%)

When we can't independently verify a dimension (an unnamed pharmacy, or no public support or shipping record), we score it a neutral 5, not a low mark: we don't penalize a provider for what we couldn't check, but an unverified dimension can't lift it above one that discloses. A documented problem (a refund or cancellation trap, an FDA action, a complaint pattern) is verified, and scores low.

Offering the FDA-approved brand too is the safest thing a compounded provider can do, so we flag it with its own prominent badge. Any provider with an FDA warning letter, fabricated testimonials, or a documented harm pattern is excluded from this list entirely, regardless of its rubric score.
