A once-weekly GLP-1 receptor agonist. Sold as Wegovy (FDA-approved for chronic weight management) and Ozempic (FDA-approved for type-2 diabetes, often prescribed off-label for weight). An oral 25 mg Wegovy tablet was FDA-approved in December 2025.
The brand drug (Wegovy) is FDA-approved and dispensed through normal pharmacy channels. Compounded semaglutide is NOT FDA-approved, so weigh that route on safety and transparency, not just price.
In the pivotal STEP 1 trial (NEJM, 2021), semaglutide 2.4 mg produced a mean −14.9% body-weight change at 68 weeks versus −2.4% on placebo, and 86% of participants lost at least 5%. The oral 25 mg version showed about 16.6% in the OASIS 4 trial. These trials were funded by Novo Nordisk, the manufacturer.
In the head-to-head SURMOUNT-5 trial (NEJM, 2025), semaglutide produced −13.7% versus tirzepatide's −20.2% at maximum tolerated doses, so tirzepatide edged it on weight loss, though both are highly effective.
