GLP-1 in Indiana: coverage & how to get it

Indiana Medicaid does not cover GLP-1s for weight loss

As of April 2026, Indiana Medicaid does not cover GLP-1 medications for the obesity indication, like most states (only 13 do). Important exception: if you have type-2 diabetes, Medicaid coverage of GLP-1s is far broader and Indiana likely covers it for that diagnosis.

Indiana has expanded Medicaid, so adults earning up to about 138% of the federal poverty level generally qualify for the program.

The diabetes route in Indiana

Indiana Medicaid does not cover GLP-1s for weight loss, but like nearly every state it covers them for type-2 diabetes. If you have type-2 diabetes or prediabetes, ask your clinician whether a GLP-1 fits that diagnosis. This is policy reporting, not medical advice.

What Indiana's Medicaid drug list says

Indiana Medicaid (Indiana Health Coverage Programs/IHCP) covers GLP-1 receptor agonists ONLY by prior authorization for qualifying medical indications, NOT for weight loss alone. The official OptumRx PA criteria document, "Criteria for Indiana Medicaid GLP-1 Receptor Agonists and Combinations" (Prepared for State of Indiana by OptumRx), explicitly heads both Zepbound (tirzepatide) and Wegovy (semaglutide) sections "(NOT COVERED EXCLUSIVELY FOR WEIGHT LOSS)." Coverage requires a qualifying diagnosis: type 2 diabetes (prior metformin trial >=90 days within past 120 days or documented intolerance/contraindication, plus baseline HbA1c within past 90 days), MASH/MASLD (with biopsy/imaging scores), moderate-to-severe obstructive sleep apnea in members with obesity (BMI >=30, Zepbound), or cardiovascular disease risk reduction in obese/overweight members (Wegovy; BMI >=27, age >=45, prior MI/stroke/PAD). Most agents require member age 18+. Initial authorizations are granted for up to 6 months unless otherwise specified. Chronic weight management/obesity as a standalone indication is not a covered use.

On Medicare in Indiana?

Traditional Medicare has generally not covered GLP-1s for weight loss alone. But a federal Medicare GLP-1 "Bridge" demonstration starts July 1, 2026, roughly $50/month for Wegovy or Zepbound for obesity, in participating plans. Coverage can also apply when the drug is prescribed for a covered condition such as heart disease. This is national, so it applies to Indiana residents on Medicare the same as anywhere else.
