Iowa Medicaid does not cover GLP-1s for weight loss
As of April 2026, Iowa Medicaid does not cover GLP-1 medications for the obesity indication, like most states (only 13 do). Important exception: if you have type-2 diabetes, Medicaid coverage of GLP-1s is far broader and Iowa likely covers it for that diagnosis.

Iowa has expanded Medicaid, so adults earning up to about 138% of the federal poverty level generally qualify for the program.
The diabetes route in Iowa
Iowa Medicaid does not cover GLP-1s for weight loss, but like nearly every state it covers them for type-2 diabetes. If you have type-2 diabetes or prediabetes, ask your clinician whether a GLP-1 fits that diagnosis. This is policy reporting, not medical advice.
What Iowa's Medicaid drug list says
Iowa Medicaid covers GLP-1 receptor agonists only for Type 2 diabetes, NOT for weight loss. The official Iowa Medicaid Drug Prior Authorization Criteria states verbatim: "Requests for weight loss are not a covered diagnosis of use and will be denied." (Anti-Diabetics, Non-Insulin Agents section). These agents require prior authorization. For non-preferred anti-diabetic non-insulin agents (which include GLP-1s) used for Type 2 Diabetes, documentation of previous trials and therapy failures with metformin, a preferred DPP-4 Inhibitor or DPP-4 combination, a preferred Incretin Mimetic, and a preferred SGLT2 Inhibitor at maximally tolerated doses is required (overridable if medically contraindicated). Initial authorizations are approved for six months; renewals are considered individually based on documented continued improvement (e.g., HgbA1C for Type 2 Diabetes).

On Medicare in Iowa?
Traditional Medicare has generally not covered GLP-1s for weight loss alone. But a federal Medicare GLP-1 "Bridge" demonstration starts July 1, 2026, roughly $50/month for Wegovy or Zepbound for obesity, in participating plans. Coverage can also apply when the drug is prescribed for a covered condition such as heart disease. This is national, so it applies to Iowa residents on Medicare the same as anywhere else.
