Michigan has expanded Medicaid, so adults earning up to about 138% of the federal poverty level generally qualify for the program.
The diabetes route in Michigan
The coverage above is the weight-loss (obesity) indication. Like nearly every state, Michigan Medicaid also covers GLP-1s for type-2 diabetes, a separate and broader pathway if you have that diagnosis. This is policy reporting, not medical advice.
What Michigan's Medicaid drug list says
Effective Jan 1, 2026, Michigan Medicaid (MDHHS) covers anti-obesity GLP-1 agents (Wegovy, Wegovy HD, Saxenda/liraglutide, Zepbound) for the weight-loss (non-diabetes) indication only with prior authorization and only as a non-preferred option. Initial criteria require: prescriber attestation of documented failure of all other clinically appropriate weight-loss interventions; trial/failure (or allergy/contraindication/intolerance) of all five preferred non-GLP-1 anti-obesity agents (orlistat, phentermine, benzphetamine, diethylpropion, phendimetrazine; phentermine/topiramate=Qsymia is preferred); adult baseline BMI classified as morbidly obese (>= 40 kg/m2); and prescriber attestation that the GLP-1 is used only as a measure to avert the need for higher-cost bariatric surgery. Authorization is 6 months initial/renewal.

On Medicare in Michigan?
Traditional Medicare has generally not covered GLP-1s for weight loss alone. But a federal Medicare GLP-1 "Bridge" demonstration starts July 1, 2026, roughly $50/month for Wegovy or Zepbound for obesity, in participating plans. Coverage can also apply when the drug is prescribed for a covered condition such as heart disease. This is national, so it applies to Michigan residents on Medicare the same as anywhere else.
How to get GLP-1 in Michigan
