GLP-1 in Tennessee: coverage & how to get it
What Tennessee Medicaid covers for weight loss (as of April 2026), how the new Medicare bridge fits in, and every legitimate route to the medication: the programs we recommend, buying direct, and the compounded route. We report the policy; we don't give medical advice.
Tennessee Medicaid covers GLP-1s for weight loss
Tennessee is one of just 13 states whose Medicaid program covers GLP-1 medications for the obesity indication. Coverage is typically behind prior authorization, so your prescriber will need to document that you meet the state's criteria. You may pay little or nothing for the drug if you qualify. Confirm the specifics with your plan.

Who qualifies for Medicaid in Tennessee
Tennessee has not expanded Medicaid, so many low-income adults do not qualify for the program at all. If you are not Medicaid-eligible, the realistic routes are a commercial or employer plan, the Medicare Bridge if you are 65 or older, or paying cash.
The diabetes route in Tennessee
The coverage above is the weight-loss (obesity) indication. Like nearly every state, Tennessee Medicaid also covers GLP-1s for type-2 diabetes, a separate and broader pathway if you have that diagnosis. This is policy reporting, not medical advice.
What Tennessee's Medicaid drug list says
TennCare (Tennessee Medicaid) covers GLP-1 agonists for Obesity Management Therapy with prior authorization. Preferred obesity agents are Wegovy and Zepbound (Saxenda/liraglutide are non-preferred and require trial/failure of two preferred agents). Per the official TennCare GLP-1 Agonists PA Form (Rev 04/2026): for patients over 18, eligibility requires an initial BMI greater than 30 kg/m2, OR BMI greater than 27 kg/m2 with a weight-related comorbidity (e.g., hypertension, dyslipidemia, diabetes, coronary heart disease, MASH/NASH, obstructive sleep apnea); for patients under 18, BMI greater than the 95th percentile standardized for age and sex. The prescriber must attest that the patient is participating in complementary nutritional and lifestyle changes.

On Medicare in Tennessee?
Traditional Medicare has generally not covered GLP-1s for weight loss alone. But a federal Medicare GLP-1 "Bridge" demonstration starts July 1, 2026, roughly $50/month for Wegovy or Zepbound for obesity, in participating plans. Coverage can also apply when the drug is prescribed for a covered condition such as heart disease. This is national, so it applies to Tennessee residents on Medicare the same as anywhere else.
How to get GLP-1 in Tennessee
If you qualify for Tennessee Medicaid coverage, ask your prescriber to bill it first. If you pay out of pocket or are not eligible, the telehealth programs we recommend operate in most states. Each is scored against our 31-point methodology, and no provider pays us anything.
Other above-board routes: buy the brand drug direct from the manufacturer (NovoCare / LillyDirect, roughly $199–$499/mo), or weigh the cheaper but not-FDA-approved compounded route. Compare the real all-in monthly cost of every option on our comparison page and price index.
How we stay independent
State coverage changes fast, and several states added or dropped GLP-1 coverage across 2025–2026. We date every figure and link the primary source; if something here is out of date, we log the fix publicly. Tell us. Provider rankings are locked against our methodology, and we take no money from any provider we cover (what our independence costs us). This is policy reporting, not medical, legal, or eligibility advice. Confirm coverage with your own plan and clinician.
