West Virginia Medicaid does not cover GLP-1s for weight loss
As of April 2026, West Virginia Medicaid does not cover GLP-1 medications for the obesity indication, like most states (only 13 do). Important exception: if you have type-2 diabetes, Medicaid coverage of GLP-1s is far broader and West Virginia likely covers it for that diagnosis.

West Virginia has expanded Medicaid, so adults earning up to about 138% of the federal poverty level generally qualify for the program.
The diabetes route in West Virginia
West Virginia Medicaid does not cover GLP-1s for weight loss, but like nearly every state it covers them for type-2 diabetes. If you have type-2 diabetes or prediabetes, ask your clinician whether a GLP-1 fits that diagnosis. This is policy reporting, not medical advice.
What West Virginia's Medicaid drug list says
Per the WV Medicaid Preferred Drug List and Prior Authorization Criteria (effective 1/1/2026), there is no GLP-1 coverage for weight loss/obesity; no obesity-indication coverage class exists. GLP-1 drugs are covered only for specific non-obesity indications with prior authorization: Zepbound (tirzepatide) is listed under "Obstructive Sleep Apnea Agents" (PA for OSA), and Wegovy is listed under "MACE Reduction Agents, GLP-1 Agonists" (PA for cardiovascular risk reduction) and also under MASH. GLP-1 agonists for Type 2 diabetes (Ozempic, Mounjaro, Rybelsus) require PA, and DPP-4 inhibitors will not be approved in combination with a GLP-1 agonist. On Medicare in West Virginia?
Traditional Medicare has generally not covered GLP-1s for weight loss alone. But a federal Medicare GLP-1 "Bridge" demonstration starts July 1, 2026, roughly $50/month for Wegovy or Zepbound for obesity, in participating plans. Coverage can also apply when the drug is prescribed for a covered condition such as heart disease. This is national, so it applies to West Virginia residents on Medicare the same as anywhere else.
