Step therapy

An insurance rule that requires you to try (and fail on) one or more cheaper or preferred medications before the plan will cover the one your prescriber wants, sometimes called "fail first." It's a common obstacle for GLP-1 coverage. Documenting the alternatives you've already tried and why they didn't work is the path through it.
